Targeted Case Management

Getting services when you have a mental health condition means dealing with a lot of systems at once. Adult & Youth Targeted Case Management gives you one person whose job is to work those systems with you.

Couple reviewing behavioral health treatment plan and paperwork together

That covers healthcare, housing, food assistance, transportation, employment services and disability benefits.

If you see several providers or agencies, your case manager keeps them talking to each other so nothing falls through the gaps.

You and your case manager write a plan around what you want to achieve and the steps to get there, then check in on it as you go.

Targeted Case Management is available to adults with serious mental illness in our service area, including people who have had a hard time sticking with services before.

What to Expect

You meet one to four times a month, at the office, at home or in the community. Between meetings your case manager makes referrals and deals with other agencies on your behalf.

Over time the aim is for you to need the case manager less. The plan changes as your situation does.
