State General Funds Case Management
State General Funds (SGF) case management is short-term, state-funded help for adults with an intellectual or developmental disability who need support now, not after a waiver comes through.

It fills the gap for people who aren’t yet on a Medicaid waiver: an urgent situation, a long waitlist, or a short stretch where someone needs services coordinated.
SGF case managers help with medical appointments, housing and employment resources, and coordination between providers and agencies, with the aim of getting the person’s situation stable and the right supports in place.
Typical situations: a living arrangement that has suddenly fallen through, a caregiver who can no longer provide support, or someone leaving a hospital or institution who needs community services lined up.
SGF is funded by the Commonwealth of Kentucky and available to eligible adults in our seven-county area. Because it is short-term, it can usually start faster than a waiver program. The case manager can also help with the longer-term options, the Michelle P Waiver and the Supports for Community Living Waiver, and with their applications.
If you or a family member needs case management now, call (270) 689-6879 or contact us online.

What to Expect
The process moves quickly. After an initial assessment, your case manager starts arranging services right away: medical providers, community resources, whatever is needed to stabilize things.
If you need case management for the long term, the case manager helps you move onto the right waiver program.
