What to Expect with IDD Services

Disability services involve a lot of agencies, forms and waiting lists. Here is how the process goes when you contact RiverValley for IDD case management, so you know what is coming.

Clinician speaks with a mother and her young daughter during an office visit

Call Point of Entry at (270) 689-6879 and say you are interested in IDD case management. They will ask a few questions and connect you with the IDD team.

Step 1: Initial conversation. Someone from the IDD team talks with you, or with the family member or guardian, about the situation and what you are looking for. It is informal, and there is nothing to prepare.

Step 2: Eligibility and documentation. Depending on the program, you may need proof of disability, such as a psychological evaluation, an individualized education plan (IEP) or physician’s records. Medicaid waiver programs like the Michelle P Waiver and the SCL Waiver also have Medicaid financial requirements. The team tells you exactly what is needed and helps you gather it.

Step 3: Assessment and service plan. Once eligibility is confirmed, your case manager assesses needs, strengths, preferences and goals, and the two of you write a service plan naming the specific supports that will help you or your family member live as independently as possible.

Step 4: Coordination. From then on your case manager is your main contact. They coordinate with providers, check that services are actually being delivered, push back on your behalf when they aren’t, and update the plan as needs change. You have regular check-ins.

In an emergency: if there has been a sudden change in living arrangements, a caregiver can no longer cope, or there is a behavioral emergency, our State General Funds (SGF) program can provide short-term case management quickly, even while a waiver application is pending. Call (270) 689-6879.
