HCBS Case Management

Home and Community Based Services (HCBS) case management helps older adults and people with physical disabilities get the services they need to keep living at home instead of in a nursing facility.

It is for adults 65 and older, and for adults and children with physical disabilities.

Your case manager assesses your needs, writes a service plan and arranges the services in it: adult day health care, personal care assistance, respite for family caregivers, home modifications and other in-home supports.

They then keep checking that the services still fit, and adjust the plan when your situation changes. That is what keeps a gap in care from turning into a hospital stay or a move to a facility.

HCBS is a Medicaid program. To qualify you must meet Medicaid financial requirements and have a level of need that would otherwise require institutional care. Our team can help with the application.

We work with your doctors, hospital social workers and family to put the right combination of services together.

What to Expect

It starts with an assessment of your needs, living situation and goals. You, your family and the case manager then write a service plan naming the supports and the providers who will deliver them.

Your case manager stays in regular contact and updates the plan as things change. Case management through this program costs you nothing.
